- To promptly notify you of changes in your patient’s health status as necessary so that treatment modifications can be made to decrease the risk of re-hospitalizations.
- To do whatever we can to save you and your staff time.
- We start all new patients within 24 to 48 hours of receiving the referral. Physicians may refer patients to physical therapy upon discharge from an acute or intermediate care facility. We will work closely with your patient’s insurance agency to identify their individual coverage.

To make a referral, simply call us at (513) 433-1117, or fax the referral to (513) 422-1119. We accept Medicare and most private insurance carriers. Our staff will work closely with your patient’s insurance company to identify their individual coverage.
